New Information has been provided to us by the NHS on the planned vaccination service for 12-15 year olds.
The following consent form needs to be completed, without this form your child will not be vaccinated.
24th Jan 2022 – Postponed Service
Due to unforeseen circumstances, the NHS team due into school to conduct the vaccinations tomorrow have been cancelled. The date of the vaccinations will be rearranged when the team can come into school to deliver the vaccinations.

Following an update from the vaccination service, please note that all parents who have registered an interest to have the COVID vaccination on Tuesday 25th January must now complete an online consent form. This online form replaces the previous paper form. The online form must now be completed for your child to receive their 2nd or 1st vaccination. This update is due to the NHS Vaccination service updating its systems.

18th Jan 2022 – Updated Information
Please find the additional update from the Vale of Evesham Primary Care Network regarding the planned vaccination programme in school for Tuesday 25th January 2022.
- If a child has had their 1st vaccination in surgery then they shouldn’t have their 2nd jab in school – due to difficulties around confirming when their 1st jab happened.
- Students can have their 2nd jab in school if they had their 1st jab in school.
- Any between 12-15 who has not had their 1st jab, can have this on 25th January.
- Any student who has had Covid within 12 weeks of the 25th January, cannot have the vaccination in school.
- A NEW consent form has been developed and will need completing prior to the vaccination on 25th January. The current consent form being issued to students is obsolete and will not be accepted by the NHS.
A new consent form is to follow once issued by the NHS.
